This summary is intended to give you a handy overview of our current Plans. Please consider this information, along with our core Values of Commitment, Leadership, Integrity and Creativity, when you consider your relationship with Luck Stone. In general, Hourly Associates are eligible for benefits after 60 days of service and Salaried Associates are eligible on the date of hire.
BASIC LIFE/ACCIDENTAL DEATH & PERSONAL LOSS (AD&PL) INSURANCE*
1.5 times annual base pay, up to a maximum of $500,000. Luck Stone covers 100% of the cost.
SUPPLEMENTAL LIFE AND AD&PL INSURANCE*
You may enroll for Supplemental Life and/or AD&PL Insurance from 1 to 5 times your annual base pay, up to a maximum (Basic + Supplemental) of $500,000. You pay premiums for this coverage through payroll deduction.
DEPENDENT LIFE INSURANCE*
Spouse coverage is available at the following levels: $5,000, $10,000, $20,000, $40,000, $60,000, $80,000 or $100,000. Coverage on unmarried children under age 19 (25 if full-time student) is available at levels from $2,000 to $10,000. You pay premiums for this coverage through payroll deduction.
* IMPORTANT NOTE
You must provide proof of good health and receive approval from Aetna Insurance Company before Life coverage (Basic + Supplemental) over $300,000 or Dependent spouse coverage over $60,000 can take effect. Such approval is also required if you do not enroll for Supplemental or Dependent coverage within 31 days of the date your Basic coverage began or if you increase Supplemental coverage by more than 1 times pay.
LONG TERM DISABILITY (LTD) INSURANCE
If you become totally and permanently disabled, the LTD coverage will pay you 50% of your base pay amount, up to a maximum of $8,333 per month. Payments begin after you have been totally and permanently disabled for 180 days. Luck Stone covers the cost of coverage and makes the benefit available to you on a tax-free basis. We also offer you the option of enrolling for an additional 10% of base pay protection. Proof of good health satisfactory to Aetna Insurance is required for the additional 10% coverage if you do not enroll promptly.
SHORT TERM DISABILITY (STD) PLAN FOR HOURLY ASSOCIATES
Luck Stone provides you with Short Term Disability coverage that continues 66 2/3% of your base pay while you are disabled and unable to work. Benefits start after the first 7 days of disability and you may use your Personal and/or Vacation Days to continue base pay during that period. Payments are made by Aetna Insurance and can continue for up to 26 weeks.
DISABILITY SALARY CONTINUTION PLAN FOR SALARIED ASSOCIATES
Luck Stone provides continuation of 100% of base salary while you are disabled and unable to work. Payments can continue for up to 26 weeks.
RETIREMENT PENSION PLAN
The Luck Stone Retirement Pension Plan provides a pension benefit upon retirement from the company. You will be given an annual Benefits Statement that includes pension estimates to help you see the value of the retirement financial security that is provided by this Plan. Luck Stone pays the entire cost of the Retirement Pension Plan.
401(k) RETIREMENT PLAN
You may begin pre-tax contributions to the Luck Stone 401(k) Plan once you start work. Luck Stone matches 100% of the first 5% of pay that you contribute.
MEDICAL COVERAGE
You may choose from two CIGNA medical Plans: the Network Plan (an HMO); or the Open Access Plus Plan (a POS). Unmarried dependent children are eligible until age 19, or up to age 25 if a full-time student. The cost of coverage is shared between you and Luck Stone.
Under the Network Plan, you choose a CIGNA network Primary Care Physician (PCP) and coordinate your care through that doctor. Your co-pay is $20 for PCP office visits and $35 for specialist visits. Hospital stays are covered at 90% after a $200 co-pay. Outpatient hospital care is covered at 90% after a $100 co-pay. Other co-pays are: $100 for ER visit; $50 for urgent care facility visit; $10 for generic drugs; $30 for brand name drugs listed by the Plan as "preferred" with no generic equivalent; and $45 for "non-preferred" brand name drugs. The Mail Order Rx offers a 3-month supply for an amount equal to 2 co-pays, with $0 co-pay for generics. Once your eligible out-of-pocket expenses, except office visit co-pays, reach $1,500 for a person or $3,000 for the family in a Plan Year, the Plan covers eligible expenses at 100% for the remainder of that Plan Year.
Under the Open Access Plus Plan, benefits are very similar to the Network Plan when you use providers that are part of the CIGNA Open Access Plus network. The Open Access Plus Plan gives you the option of using providers who do not participate in the network. Prescriptions and routine preventive care are only covered in-network.
For out-of-network care, a Plan Year deductible of $200 applies (not more than $400 total per family). Most items are covered at 70% after the deductible has been met. Separate deductibles of $400 per hospital stay and $200 for outpatient facility use also apply. Once your eligible non-network out-of-pocket expenses, except the Plan Year deductible, reach $3,000 for a person (not more than $6,000 total per family) in a Plan year, the Plan will waive additional separate deductibles for the remainder of that Plan year (70% coinsurance will continue to apply).
DENTAL COVERAGE
The Plan pays 100% of Preventive Services, 80% of Basic Services and 50% Major Services. A combined deductible of $50 per Plan Year applies to Basic & Major Services (not more than 3 deductibles applied per family). The maximum paid for eligible expenses each Plan Year is $1,500. Orthodontic Services for dependent children are covered at 50% up to a lifetime maximum of $1,000. Unmarried dependent children are eligible until age 19, or up to age 25 if a full-time student.
VISION COVERAGE
Coverage begins on your hire date, provided you have enrolled. Luck Stone provides a vision Plan through Vision Service Plan (VSP) that covers 1 eye exam per year for a $10 co-pay and the purchase of glasses or contacts every year. Your pre-tax monthly payroll deductions cover 100% of the Plan cost. You may contact VSP at www.vsp.com or 800-241-2448.

HEALTH CARE FLEXIBLE SPENDING PLAN
Through the Health Care Flexible Spending Plan, you can contribute pre-tax dollars to an account to be used for paying eligible health care expenses incurred during the Plan Year. The eligible expenses mirror those that are recognized by the IRS and include such items as: co-pays, coinsurance amounts, laser eye surgery, and orthodontia. Items such as cosmetic surgery, exercise equipment, health club dues, and vitamins are not eligible. You must carefully consider the amount that you will need for health care expenses, since any unused amount in your account must be forfeited and will be lost to you.
DEPENDENT CARE FLEXIBLE SPENDING PLAN
Through the Dependent Care Flexible Spending Plan, you can contribute pre-tax dollars to an account to be used for paying eligible child care expenses incurred during the Plan Year. The eligible expenses must be incurred during the Plan Year for your child under the age of 13 (over 13 if handicapped) so that you (and your spouse, if married) can work. You must carefully consider the amount that you will need for dependent care expenses, since any unused amount in your account must be forfeited and will be lost to you.
VACATION
You earn up to 10 days of vacation at the rate of 8 hours per month of service during your first year of partial service. 10 vacation days are provided for your first full year of service. You will receive 15 days of vacation when you have completed 6 years of service, 20 days when you complete 11 years of service, and 1 more day for each additional year of service after 20 years, up to a maximum of 25 days. Also, you may enroll before the start of each Plan year (Nov 1 - Oct 31) to purchase up to 5 vacation days by payroll deduction. Each vacation day equals 8 hours of time.
PERSONAL DAYS FOR HOURLY ASSOCIATES
Hourly Associates are provided with 5 Personal Days each Plan Year (Nov 1 - Oct 31).
HOLIDAYS
Luck Stone honors 8 holidays: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, the day after Thanksgiving, Christmas Eve and Christmas Day. All Associates are also provided with 2 Floating Holidays each Plan Year (Nov 1 - Oct 31).
